General Information

Becoming a Patient of Dr Hope

What To Bring To Your Appointment

Preparing For Your Apppointment

Who We Welcome at Our Practice

Dr Hope is currently accepting new patients at his private practice. He treats patients with or without private health insurance, those with active WorkCover claims, and Department of Veterans’ Affairs (DVA) cardholders.

While Dr Hope also has a public appointment at Princess Alexandra Hospital, he does not offer public or intermediate services at his private practice. To go through the public system you will need to be referred by your GP.

Booking an Appointment

To book an appointment with Dr Hope as a private patient at Spring Hill or Cleveland, please call 3834 6590 Monday to Friday, 8:30 am to 4:30 pm.

Imaging & Scans

When booking, we’ll ask if you’ve had any scans related to your condition. If not, or if more are needed (e.g. MRI or CT), we’ll give you a referral.
We recommend larger radiology providers like LUMUS, IMED, QLD X-Ray, or Qscan for the best quality. For convenience, X-ray services are available onsite at LUMUS Imaging (ph 07 3834 6125). No booking is needed for a plain X-ray, but you will need one for an ultrasound, CT, or MRI.

Dr Hope likes to be well prepared for your visit and will review your scans before your appointment. Having all the necessary imaging ready on the day means he can give you an accurate diagnosis and treatment plan straight away—saving you time and avoiding extra costs.

New Patient Form (online)

When you book your first appointment, we’ll text you a link to our online New Patient Registration Form.

When filling out the form, please have these ready:

  • Medicare card
  • Private health insurance card
  • DVA card (if you have one)
  • Workers’ Compensation claim info (if it applies)
  • Pension card (if you have one)
  • List of your current medications
  • Name of the X-ray clinic where your scans were done

Referral

You’ll need a referral to see Dr Hope so you can claim a Medicare rebate. Without one, you can still see us but won’t get a rebate. GP referrals last 12 months, and specialist referrals last 3 months.

Other Investigations

If you’ve seen another specialist (e.g. a Neurologist), please bring any relevant test results, such as nerve conduction studies. If you have a Cardiologist involved in your care and need surgery, you will require written clearance that you are fit for surgery.

List of Current Medications

Please bring a complete list of your current medications, including prescriptions, over-the-counter medicines, and supplements. Our team will let you know if any need to be stopped before surgery.
